Modafinil was first described in the 1970s as a wakefulness-promoting agent with a distinctive pharmacologic profile. It is widely used worldwide for narcolepsy, obstructive sleep apnea/hypopnea syndrome, and shift-work sleep disorder.
Modafinil is a prescription wakefulness-promoting agent (eugeroic) with a distinct pharmacology that modestly engages orexin signaling and dopaminergic systems. It differs from classic stimulants in onset and abuse potential and is commonly formulated as 100- and 200-milligram tablets.
Indications include narcolepsy, a chronic disorder characterized by daytime sleepiness and sudden sleep attacks. It is approved for obstructive sleep apnea/hypopnea syndrome to improve wakefulness in patients with sleep-disordered breathing. A third approved use is shift-work sleep disorder, where wakefulness is impaired by irregular work hours. Treatment should consider comorbidity and concurrent medications.
Typical dosing ranges from 100 to 200 mg once daily in the morning. The maximum recommended daily dose is 400 mg, and higher doses should be used only under close medical supervision. For shift-work sleep disorder, administer about one hour before the start of the shift or as directed by a clinician. Food does not markedly alter absorption, so the drug may be taken with or without food.
If a dose is missed, take it as soon as remembered unless the next dose is near. Do not double-dose to compensate. Do not exceed the prescribed daily amount. Storage should be at room temperature, away from moisture and heat; keep tablets in the original container and out of reach of children; discard after the expiration date.
Common adverse effects include headache, nausea, dry mouth, dizziness, and anxiety or insomnia. Some individuals may experience palpitations or gastrointestinal upset.
Serious reactions are rare but may occur, including severe skin rash or hypersensitivity and chest pain or signs of cardiovascular events. Mood changes and new or worsening anxiety or depressive symptoms can arise. Pregnant or breastfeeding individuals should consult a clinician before use, and hormonal contraception use may require counseling due to potential interactions.
