High blood pressure and heart failure can cause swelling, shortness of breath, or fatigue. After a heart attack, your heart may not pump as well as before. Eplerenone is used to help these problems. It is a prescription medicine that blocks the hormone aldosterone, helping your body release extra salt and water and easing your heartβs workload.
Eplerenone is a selective mineralocorticoid receptor antagonist. In simple terms, it blocks aldosterone, a hormone that can raise blood pressure and make the body hold onto salt and water. By blocking these signals, it can help lower blood pressure and reduce swelling.
Primary indications include hypertension (high blood pressure) and heart failure with reduced ejection fraction (HFrEF). If you have HFrEF, eplerenone lowers the risk of hospital visits for heart problems and can improve how you feel. It is also used after a heart attack in people with left ventricular dysfunction to reduce the risk of death and further heart problems.
Secondary indications include situations where aldosterone is high or blood pressure remains above target despite other medicines. Your doctor may add eplerenone when other heart medicines donβt fully control your symptoms. It may be used as part of a plan to protect your heart and kidneys in certain patients. Your doctor will decide if this medicine is right for you based on your blood tests and health factors.
Eplerenone blocks aldosterone receptors mainly in the kidneys and heart. This reduces salt and water retention, helping to lower blood pressure and ease the heartβs workload. It can also help prevent harmful changes in heart tissue after injury, supporting longer-term heart function.
Common side effects include dizziness or lightheadedness, especially when you stand up. You might have stomach upset or diarrhea. A key safety point is the risk of high potassium in the blood (hyperkalemia), particularly if your kidneys donβt work well or you take other medicines that raise potassium. Your doctor will check your kidney function and potassium levels while you use eplerenone.
Do not use if you have very high potassium or serious kidney disease. Tell your doctor about other medicines you take, especially ACE inhibitors, ARBs, NSAIDs, and potassium supplements. If you notice muscle weakness, a slow or irregular heartbeat, or numbness, report it to your clinician promptly.
